Patient Medical History: Allergies, Vaccines & Background

Manage a patient’s complete medical history in Turnero including allergies, vaccines, growth charts, and personal and perinatal background.

Each patient record in Turnero includes a comprehensive medical history section where doctors can record and review allergies, vaccination records, growth measurements, and personal and perinatal background information.

Allergies

1. Open the Allergies tab

   Navigate to the patient’s detail page and click the Allergies tab.

2. Add a new allergy

   Click Add Allergy to open the allergy form.

3. Fill in the allergy details

   Complete the fields described below. Name and Start Date are required; all other fields are optional.

   Type	No	Category — food, drug, environmental, etc.
   Severity	No	How severe the allergic reaction is
   Start Date	Yes	Date the allergy was first identified
   End Date	No	Date the allergy resolved, if applicable
   Description	No	Additional clinical description
   Occurrence	No	How frequently reactions occur
   Comments	No	Free-text notes

4. Save the record

   Click Save to add the allergy to the patient’s record.

To update an existing allergy entry, click the edit icon on its row. To remove an entry permanently, click the delete icon.

Vaccines

1. Open the Vaccines tab on the patient’s detail page.
2. Click Add Vaccine and select the vaccine name from the dropdown list. If the vaccine is not listed, choose the free-text option and enter the name manually.
3. Enter the date administered in the date field.
4. Click Save.

To correct a vaccine entry, click its edit icon. To remove it, click the delete icon.

Growth Chart

Track a patient’s physical development over time using the Growth Chart tab. Each entry records measurements at a specific age, and Turnero automatically calculates the BMI for that entry.

1. Open the Growth Chart tab on the patient’s detail page.
2. Click Add Entry to record a new measurement.
3. Fill in the measurement fields — Age, Weight, Height, and Head Circumference — and select the corresponding time unit (for example, months or years).
4. Click Save.

Previous entries appear in a table. Each row shows the recorded age, weight, height, head circumference, their respective percentile values, and the calculated BMI. Use the edit and delete icons on each row to update or remove a measurement.

Personal Background

The Personal Background section records prior medical conditions the patient has had. Each condition is toggled on or off with a checkbox:

Respiratory

Asthma, Pulmonological conditions, Pneumonia

Infectious / Childhood illness

Mumps, Rubella, Measles, Chickenpox, Otitis, Urinary Infections

Chronic / Systemic

Allergies, Diabetes, Hematological/Oncological conditions, Digestive conditions

Other

Psychological conditions, Accidents, Surgeries, and a free-text Other field for conditions not listed above

Perinatal Background

For pediatric patients, the Perinatal Background section records birth-related data:

Apgar 5 min	Apgar score at 5 minutes
Gestational Age	Gestational age at birth (weeks)
Pathologies	Any perinatal pathologies noted
Congenital Errors	Congenital anomalies or errors

Parents’ / Guardian Data

For pediatric patients, record parent or guardian information in the Parents tab. You can store the following for both the father and the mother:

• Full name
• Date of birth
• Blood type
• Occupation

You can also record the number of siblings the patient has.

All medical history sections are only accessible and editable by users with the Medico (Doctor) role.
